I request you to book the following date(s) in Raghavendra Mandapam for Marriage / Reception / Exhibition Meeting / Function. I have gone through the rules and regulations of the Raghavendra Mandapam and the same were explained to me in detail by the Mandapam management. I am prepared to abide by the same. I accept the rules and regulations of Raghavendra mandapam and I shall follow the same without fail.

I enclose herewith Cheque(s), as per the Payment details stated in the rules and regulations form of Raghavendra Mandapam.

Further, I agree to pay the caution deposit for additional charges 10 days prior to the date(s) booked for the functions, and agree to deduct the following from the caution deposit.

I clearly understand and agree that the expenses towards flower decoration, vessels & dining hall cleaning to be undertaken by the contractors appointed by the mandapam management are in addition to the mandapam rental and caution deposit.

I agree to pay the increased in rent / caution deposit and other charges, if any, in the event of a revision in the rental structure prior to the function.

I hereby undertake to utilize the service as per the rules and regulations of the Mandapam. In case of any default on my part or any breach by me or due to any other reason on account of which I am unable to utilize the mandapam on the booked / agreed dates, I shall not ask for the refund of any of the money I have paid.

I have read the rules and regulations of Raghavendra Mandapam and agree to abide by them.
